PPHD works to bring evidence-based healthy lifestyle support programs in the Panhandle area that help patients learn how to manage their health outside the clinical setting in their everyday lives to improve health outcomes and quality of life.
The Chronic Disease Self-Management workshop was developed at Stanford University and has many years of research behind it showing that participants who take the 6-week workshop make significant short-term improvements in pain, dependency, vitality, aspects of role functioning, life satisfaction and in self-efficacy and resourcefulness.
The workshop is called Living Well in Nebraska. The Living Well programs do not conflict with existing programs or treatment. Medical treatment is never altered. For all medical questions including physical activity and medication questions, participants are referred to their physicians or other health professionals.
Additionally, Panhandle Public Health District offers Living Well with Diabetes and Living Well with Chronic Pain workshops that are focused on those specific conditions.
Please consider referring your patients to this valuable workshop offered at no cost to them. There is a virtual workshop beginning in January that your patients could attend.
